Records Release to Have Your Medical Records Sent to GMA

Be sure to fill out a copy of the Records Release for each of your previous practitioners and send it to them in plenty of time before your appointment, so that Gordon Medical will have access to your medical records.

If you need a copy of your Medical Records from Gordon Medical, for yourself or another practitioner, fill out the Records Request form and send it to the Gordon Medical records department.

Records Copying Fees

Gordon Medical Associates follows the Medical Board of California’s recommendation in charging patients for copying their medical records, and follows HIPAA guidelines for privacy.

The costs for copying your records can add up. When requesting records, please be specific regarding labs, tests and records that you may already have copies of or may be available on your patient portal. This will help you keep costs down.

Can a doctor charge me for copies of my medical records or x-rays?

Yes, as of 2016, pursuant to Health & Safety Code section 123110, a doctor can charge 25 cents per page plus a reasonable clerical fee. For diagnostic films, such as an x-ray, MRI, CT and PET scans, you can be charged the actual cost of copying the films. This only applies if you have made a written request for a copy of your medical records to be provided to you.
